He cuts himself up, shoots himself in the head, picks at a boil on his face, cuts off his dick with scissors, hammers at a dead rat while rubbing one out, murders a jogger, rips his ass apart with a crucifix, before finally going after a woman dressed like his grandma. Or maybe it was her he was missing all the while. Who cares. Just nice practical effects.
